Provide hands-on acute care in the home setting for the Home-Based Hospital Care patient population requiring the use of advanced assessment, technical, teaching, and decision-making skills.

We are seeking a Registered Nurse (RN):

* Work directly with patients and families in the comfort of their home utilizing care plans, interventions and equipment that are modeled after an acute care inpatient unit setting.
* Perform comprehensive assessments of physical, functional, psychosocial, and cognitive status of the Home-Based Hospital Care patient utilizing interview observations and physical exam techniques.
* Assess post-acute needs and ensure that appropriate referrals to other services are made. Interdisciplinary conferencing is a key component of this role's responsibility, and thorough and timely documentation in EPIC is required.

Education

Bachelor of Science in Nursing (BSN) Preferred

Work Experience

* One year of relevant acute care or home health nursing experience Required
* Experience in teaching self-care skills to patients and families Preferred
* Recent, relevant home health experience Preferred
* Experience using EPIC and virtual health platforms Preferred
* Current IV therapy skills with knowledge of venous access devices and ambulatory pumps Preferred

Licenses & Certifications

* Licensed as a Registered Nurse (RN) in the State of WI or holds a license issued by a jurisdiction that has adopted the nurse licensure compact Required
* Current CPR/BLS Certification Required
* Current motor vehicle liability insurance in accordance with the UW Health "Motor Vehicle Policy" (Policy 1.17) and (1) A valid Wisconsin driver's license and (2) Successfully passing a driver's background check and (3) Two years of driving experience Required
* Relevant certification Preferred
