The Registered Nurse provides care to sick, disabled and/or injured persons in their home environment under the direction of a physician and in collaboration with other team members. He/she assesses individual and family health needs, collaborates in the development of a nursing care plan, carries out nursing skills, teaches health management concepts and consults appropriate resources within the organization and community.

Required Qualifications:

* Degree/diploma from accredited school of nursing.
* Washington Registered Nurse License upon hire.
* National Provider BLS - American Heart Association within 30 days of hire.
* National Provider Identifier (NPI) number within 30 days of hire.
* 1 year of experience in acute care.

Preferred Qualifications:

* Bachelor's Degree - BS Degree in nursing from accredited school of nursing with public health preparation.
* 3 years of experience in home health, acute care or a skilled nursing facility.
* Recent home health, acute or extended care experience
