RN - Registered Nurse - Navigator - Cardiology Medical Group

Partners with physician, clinical staff, and hospital in the assessment, planning and implementation of services provided by the outpatient and inpatient cardiology service line.

Regularly assesses educational needs of the patient and family members in an effort to provide appropriate instructional materials and recommendations that will enhance care for the patient.

Assesses the patient's plan of care and develops, implements, monitors, and documents the utilization of resources and progress of the patient through their care, facilitating options and services to meet the patient's health care needs.

Responsible for development and oversight of care coordination efforts provided to the patient.

Implement and ensure compliance of treatment plans developed by providers for patients under the provider’s care.

Maintains process for assessment and identification of high-risk patients, making the provider aware and facilitating the necessary referrals to maximize continuous quality and coordination of care to the patient.

Attends relevant meetings and keeps current on industry practice standards and relevant literature.

Demonstrates self-directed excellent communication and customer service skills. Able to analyze/interpret data and relate results to the clinical performance of the specialty area.
