We go beyond primary care with a full spectrum of services including cardiovascular health services, behavioral health, allergy testing and immunotherapy, in-house lab testing, imaging, chronic disease management and other specialty health services. Optima Medical is currently seeking a Psychiatric Nurse Practitioner to provide primary care to psychiatry and at-risk patients through virtual communication while working remotely.

*Must reside in Arizona or open to relocating to Arizona to be considered for the role*

Collaborative Care Model (CoCM):

CoCM is a team-based approach to mental health that involves collaboration between primary care providers, behavioral health specialists, and Psychiatric providers to deliver integrated and comprehensive care to individuals with mental health conditions. The goal of CoCM is to improve access to mental health services through community-based recourses, enhance patient outcomes, and provide more holistic and coordinated care.
Providing Psychiatric support and consultations as a member of our Behavioral Health Collaborative Care Team historyVirtually consults with patients to address Behavioral Health concernsUtilizes quality improvement date to enhance patient careWeekly meeting with CMO and Behavioral Health Care Manager LeadWill work directly with care managers and referring PCP's assist.
Job Qualifications:
Bachelor's degree in nursing followed by a Master's degree in Psychiatric or Mental Health Nursing from accredited school for professional nursing trainingPossess strong understanding of societies and their impact on mental health; ability to solve problems, monitor and examine patientsBe compassionate, sensitive, and possess strong reasoning and thinking skillsExcellent written and verbal communication skillsAt least 1 year of clinical work experience as Psychiatric Nurse Practitioner At least 1 year of EMR experience; strong>
