Description:

Join our team Galloway Ridge is an upscale continuing care retirement community located between Chapel Hill and Pittsboro in Chatham County, NC. The Galloway Ridge Clinic is seeking an LPN to work Full-time Monday-Friday 8:00am-4:30pm. No weekends are required, but Clinic Nurse will be required to work one Galloway Ridge recognized holiday each year. The Clinic Nurse will collaborate with medical providers, specialists and interdisciplinary team to maintain efficient services to meet needs of Galloway Ridge residents and employees in onsite clinic.

Additional responsibilities include but are not limited to:

Gather/obtain relevant resident data (ex: vital signs, blood sugars, weights, heights) and present to medical providers at clinic during scheduled and acute visits. Assist residents as required during clinic visits.
Schedule appointments to equally distribute workload of providers and optimize efficiency of clinic while meeting needs and accommodating requests of residents. Utilize electronic scheduling system for optimal access with practitioners and nurses.
Provide nursing services as outlined by MD/NP orders in accordance with regulations, facility policies/procedures and per nursing standards of care. Enter MD orders into electronic record as required. Hours may fluctuate to meet the needs of clinic as outlined by the Director of Clinical and Navigation Services.
Keep medical information up-to-date, accurate, and confidential.
Document all communications with providers, residents and family members as well as services rendered into electronic record system.
Complete billing logs/reports as required
Consistently demonstrates hospitable behavior and core values of Caring, Empowering, Integrity, and Commitment. Completes screenings for new hire candidates as required. Completes all necessary paperwork accurately and thoroughly. Returns necessary paperwork to HR in a timely manner.
Complete lab requisitions and other reports for completion of diagnostic testing. Assist with scheduling outside appointment to specialists, outpatient clinics, etc. Follow up on receipt of lab results, diagnostic testing, x-rays, scans, etc. and report findings to medial provider in a timely manner for optimal quality and care continuity.
Maintain clean clinic environment, adhering to safety and infection control policies and procedures and standards of practice. Keep clinic stocked with necessary supplies to meet needs of residents.
May provide backup services to Community Health Navigator and others as instructed by Assistant Director of Clinical Services.
All other duties assigned include, but not limited to, enhancing the Resident Experience.
Requirements:
Must be able to read and write clearly.
Must be able to communicate in courteous, friendly manner.
Must be able to use computer to complete and read electronic documentation and scheduling program.
Must have ability to solve practical problems and deal with a variety of concrete variables where limited standardization exists.
Ability to interpret variety of instructions furnished in written, oral, diagram, electronic or scheduled form required.
Conflict resolution skills required.
PDSS and NCID required; can be obtained upon employment
LPN License.
Basic Life Support/AED Certification.
