Requirements:
- Required Certifications: BLS, BSW
- Required Skills: Emergency Department, Medical Surgical unit, Short Term Acute Care Inpatient Medical Social Worker, Assess response to treatment, Develop plan of care to meet patient/caregiver needs, Development of problem list, Establish and revise goals, Review of Health history, Collaborative care, Coordinates with internal and external team members, Documents pertinent information to facilitate care coordination, Participates in care conference activities, Refers to rehab and long term care facilities, Documentation according to plan of care, Documentation of care delivery, plan, goals, etc, Documentation of patient/caregiver teaching/learning, Documentation reflects understanding of coverage criteria, Documents patient response, Advance Directives, Medicaid, Medicare, Prior Authorization, Social Security, HCBS (Home and Community Based Services), Medicare waiver, Adult protective services, Child protective services, Emergency detention, Guardianship, Power of Attorney, Arthritis, Cancer, CHF, COPD, Dementia, Diabetes, HIV/AIDS, Immune Deficiency, Neurological disorders, Paralysis, Psychiatric Disorders, Stroke, Case management, Benefits Eligibility, Discharge Planning
Job Details:
- Pay: $2326/week - $2556/week
- Assignment Length: 13-week assignment
- Shift: 5, 8-Hour 08:00 - 16:30
- At least one year of experience required
