Apply for the HYBRID MDFT Therapist role at Hope For Youth, Inc Overview This full‑time, 35‑hour hybrid position supports the PINS Diversion program by delivering Multidimensional Family Therapy (MDFT) to youth at risk in Nassau County. The role focuses on preventing out‑of‑home placements, teaching families problem‑solving skills, and connecting families to community resources. Key Responsibilities Adhere to agency policies, the employee manual, program guide, and applicable codes of conduct. Provide support to clients and families and equip them with knowledge, skills and tools to maintain children in their homes. Achieve successful outcomes in line with NYS, county, and HFY policy and procedure. Perform additional duties necessary to meet client, program, agency, oversight‑agency, and regulatory obligations. Time Spent / Clinical Work Work as a licensed therapist (LMSW, LCSW, LMHC, or LMFT) and develop comprehensive treatment plans for youth with high acuity behavioral and trauma needs per CANS‑NY. Provide individual clinical services to a caseload of up to 12 clients at any time, while the program serves up to 50 clinical cases total annually. Complete a 3‑ to 6‑month MDFT training program (30‑60 sessions over 6 months; clients seen 1‑4 times a week as needed). Administer home, community, or office‑based interventions. Conduct collateral contacts in schools and community locations. Prepare psychosocial assessments, treatment plans, progress notes, interdisciplinary team conference documentation, and other case‑related information. Meet physical requirements such as walking, standing, sitting, bending, kneeling, squatting, climbing stairs, reaching overhead, and maintaining reliable transportation. Qualifications License (LMSW, LCSW, LMHC, or LMFT) required. Spanish and English speaking required. Experience preferred; training in the MDFT model will be provided. Valid NYS driver’s license in good standing. Additional Abilities Excellent written, verbal, and computer skills. Flexible schedule to accommodate program needs. Active participation in treatment team, case review, and other meetings. Collaborate with referral resources and agencies working with families and court. Work as a member of an interdisciplinary team. Provide support for other projects and locations; travel as needed. Knowledge of child-serving systems. Experience working with adolescents.
