Join to apply for the Behavioral Health - Case Manager II role at Elevance Health. Location: Virtual. Candidates must be able to visit the facility twice per week. Locations are Chestnut Hill or North Attleboro, MA to meet with Social Work teams and patients for discharge planning. Must live in Massachusetts. The Behavioral Health - Case Manager II is responsible for performing case management telephonically and/or by home visits within the scope of licensure for members with behavioral health and substance abuse needs. Subject matter expert in specialized clinical areas such as Eating Disorders, Maternity Alcohol/Drug, Autism Spectrum Disorders, etc. How You Will Make An Impact Responds to more complex cases and account specific requests. Uses appropriate screening criteria and clinical judgment to assess member needs. Conducts assessments to identify individual needs and develops specific care plans to address objectives and goals. Monitors and evaluates the effectiveness of care plans and modifies them as needed. Supports member access to appropriate quality and cost‑effective care. Coordinates with internal and external resources to meet identified needs and collaborates with providers. Serves as a resource to other behavioral health case managers. Participates in cross‑functional team projects and initiatives. Minimum Requirements MA/MS degree in social work counseling or a related behavioral health field (or nursing) and at least 3 years of clinical experience in social work counseling with a broad range of complex psychiatric and substance abuse treatment; or equivalent education and experience. Current, active, unrestricted license (RN, LCSW, LMHC, LICSW, LPC, LMFT, LMSW, Clinical Psychologist, etc.) as permitted by state law. Previous experience in case management and telephonic and/or in‑person coaching with members having complex psychiatric/substance abuse and/or medical disorders. Managed care experience required. Preferred Skill, Capabilities, And Experiences Experience in health coaching and motivational interviewing techniques. Salary range for this position: $68,400 to $112,860.
