RN Case Manager - Hospice Join to apply for the RN Case Manager - Hospice role at LHC Group at Louisiana Hospice and Palliative Care, a part of LHC Group. Summary We are hiring for a Registered Nurse Case Manager. As a Hospice RN, you will develop trusting relationships as an end-of-life care expert, be valued by patients and families, and benefit from employee-focused wellness and support programs. Responsibilities Assumes full nursing responsibility for the delivery of the Plan of Care for each hospice patient. Makes the initial nursing evaluation to determine eligibility for hospice services within forty-eight (48) hours of referral. Identifies patient/family physical, psychosocial, and environmental needs and re‑assesses as needed, no less than every fifteen (15) days. Initiates and coordinates the plan of care. Documents problems, appropriate goals, interventions, and patient/family response to hospice care. Collaborates with patient/family, attending physician, and other IDG members in providing care daily. Instructs and supervises patients/families in self‑care techniques when appropriate. Maintains accurate and relevant clinical notes regarding the patient's condition. Education & Experience Experience Requirements: One year of clinical experience. License Requirements: Current RN licensure in the state of practice and current CPR certification. Must have a valid driver’s license, vehicle insurance, and access to a dependable vehicle or public transportation. Additional State Specific Requirements LA: Minimum two years full‑time experience as an RN, or two years full‑time experience in hospice care as a licensed practical nurse may substitute.
