General Summary
  • The goal of navigation is to reduce cancer morbidity and mortality by eliminating barriers to cancer prevention, screening, and cancer care. The Nurse Navigator begins a relationship with the patient at the time of cancer diagnosis and supports them throughout their cancer journey. The core responsibilities of the Nurse Navigator role include the provision and coordination of care, communication, and education. In addition, the Nurse Navigator is responsible for the management and promotion of evidence-based cancer screening and prevention programs. The scope of responsibility for the Nurse Navigator will be to serve the community and patients across the service areas for Phelps Health while supporting the Mission, Vision and Values of the organization.

Essential Duties and Responsibilities
  • Provides patients with information regarding available resources and options to facilitate shared decision making with their care providers. Acts as a clinical liaison for newly diagnosed cancer patients and other patients, as needed.
  • Provides hands on clinical care within professional scope of practice to include patient care during oncology, surgical and/multidisciplinary clinics and documents in the EHR, as appropriate. Visits with oncology inpatients as needed to evaluate specific patient navigation. Assists clinicians with management of the cancer survivorship program.
  • Collaborates with other disciplines in forming a timely and appropriate plan of care. This includes active participation in weekly tumor conferences, cancer committee and other quality programs. The Nurse Navigator is responsible for the Nurse Navigator CoC Report. Establishes and maintains rapport with referring physicians and offices.
  • Collaborates with and offers support to the DDCI Core Ancillary Team (i.e. Dietitian, Social Worker, and Financial Counselor).
  • Manages the daily operations and workflows of the Lung Cancer Screening Program, including follow up management and all aspects of the CT Lung database (data entry, statistical reporting and enhancement).
  • In accordance with criteria from NCCN and the United States Preventative Service Task Force, enrolls patients into LDCT Lung Cancer Screening Program and responds to questions appropriately. Communicates with referring physicians regarding appropriateness for screening program(s) and coordinates information with referral services.
  • Provides patient and community education for cancer prevention and treatment. Conducts community outreach, education, and engagement activities while collaborating with community organizations. Directs individuals to available resources and supportive services. Provide updates on screening and prevention program activity, statistics and recommendations for program enhancements.
  • Coordinates other core cancer services such as support groups and educational programs. Maintains the DDCI Resource Room. This position serves as the primary contact for the American Cancer Society Representative and coordinates the Define Your Shine Program. Develops and manages processes that directly improve care management and care coordination for the oncology patient population.

Education
  • Graduate of an accredited nursing program required. BSN preferred.

Work Experience
  • Minimum of 3 years general RN experience preferred
  • Computer proficiency in MS Office and database systems is required

Certification/License
  • Current MO RN License or RN Compact State Licensure.
  • Oncology Nurse Certification Preferred.
  • BLS Required
  • Current/Valid Driver's license

Mental/Physical Requirements
  • Considerable mental concentration required to complete duties. Must be able to sit and type utilizing a computer. Sitting or transporting self throughout facility 60% of time. Light lifting (up to 15 lbs.) required. Strong communication, organizational and interpersonal skills.

Working Conditions
  • Potential exposure to infectious disease. Slight possibility of exposure to radiation, anti-neoplastic agents, and hostile persons. Minimal physical discomfort. Frequent auditory distractions. Works with sharps and other potentially biohazardous material at times.
