Nurse Case Manager Position in Home Care

Alternate Solutions Health Network LLC is seeking a qualified Registered Nurse Case Manager to join our team in providing high-quality patient care in a home care setting.
About the Role:This is an exciting opportunity for a Registered Nurse to work autonomously and provide clinical care to patients in their homes. As a Registered Nurse Case Manager, you will be responsible for assessing patient conditions, developing individualized care plans, and collaborating with healthcare teams to ensure optimal patient outcomes.
Key Responsibilities:
- Assess patient conditions and develop individualized care plans
- Provide clinical care to patients in their homes
- Collaborate with healthcare teams to ensure optimal patient outcomes
- Monitor patient conditions and adjust care plans as needed
- Administer medication as prescribed by physicians
To be successful in this role, you will need:
- A current Registered Nurse license in the state of employment
- A minimum of one year of experience as an RN in an acute care setting
- Valid driver's license and auto insurance
- Ability to perform all physical demands of the job
