Job Responsibilities:

The Household Coordinator/CNA Supervisor is a blended role whose primary responsibility is to oversee the daily operational function of the ‘household’ within the nursing facility. This role oversees the social function of the household, including activities. This position serves as a mentor/leader and partners with the Clinical Coordinator in the coordination of care of the residents within the household.

What you will do:

Develops positive resident relationships, exploring approaches to aging and supporting families.
Addresses and documents any special psychological concerns (death in family, health decline etc.).
Maintains individualized psychosocial assessment.
Evaluates personal and emotional concerns.
Assists with the completion of an individualized activity assessment.
Maintains household activity calendar and coordinates activities and ensure resident appointments are kept.

What we require:

Valid LPN Licensure
One (1) year experience in long term care, as an LPN.
Must have critical thinking skills and decision making ability.
Must have excellent leadership skills and abilities.
Excellent communication skills in order to effectively communicate with residents, families, and staff.
