Generous Paid Benefit Time. Responsibilities Coordinate the hospital‑wide case management program. Oversee review of patients’ medical records to ensure proper utilization of hospital services. Assist case managers in providing timely coordination of care and discharge planning. Ensure screening of all admissions to determine appropriate level of care. Supervise case managers, overseeing daily operations of the case management department to facilitate optimal financial and clinical outcomes. Handle fiscal planning and regulatory compliance. Analyze current systems and variances to identify opportunities for improvement and promote quality of care through collaboration with the interdisciplinary team. Qualifications Active RN license in the state where the hospital resides; BSN/MSc preferred. Certification in an approved Case Management Program is preferred. Current BLS certification required. Licensed Social Worker (LSW) accepted in lieu of RN license for facility acquisitions. Three to five years of case management experience required. Prior experience in IRF or LTACH setting preferred. Prior management experience preferred.
