Job Description

Job Summary:

The Hospice Social Worker BSW provides medical social services, bereavement services and coordinated volunteer activities. The Hospice Social Worker BSW provides a wide range of services to terminally-ill patients and families while consistently exercising judgment and discretion to determine the patient and family's unique spiritual/ cultural beliefs, strengths, coping skills, and pre-bereavement and anticipatory grief needs. Community outreach, community education, and bereavement support group services are also functions of the Hospice Social Worker BSW. Adherence to company policy, procedures, and regulatory compliance is required, and travel will be needed as necessary.

Essential Job Duties:
  1. Interprets hospice program/services for patients and families, reviews referrals, and initiates psychosocial assessments to identify patient/family needs, resources, and acceptance of Hospice
  2. Participates in the Interdisciplinary Group (IDG) meetings assisting in the development of a Comprehensive plan of care; communicates and interprets patient/family needs and problems to the IDG
  3. Maintains ongoing, supportive counseling with patients/families, assisting them with difficulties, and meets social and emotional needs relevant to advanced illness, death and bereavement.
  4. Makes appropriate referrals to community agencies and/or community resources. Also advises staff/volunteers regarding services available to patients and families.
  5. Provides bereavement counseling
  6. Addresses high- risk needs. Assumes timely ongoing assessment of individual social work, bereavement and or volunteer needs.
  7. Appropriately assesses social work, bereavement / volunteer needs and evaluates the effectiveness of interventions, adapting the plan of care accordingly.
  8. Demonstrates timely, accurate, and appropriate documentation of social work, bereavement / volunteer contacts.
  9. Serves as a resource to volunteers and staff

Secondary Job Duties That May be Reassigned:
  1. Assists with and participates with community events sponsored by Good Samaritan Hospital/Hospice
  2. Assists to maintain a positive working environment by working together as a team member
  3. Makes scheduled visits and assures coordination with the Nurse Manager
  4. Recognizes individual role as a representative of the agency and conveys a positive organizational image
  5. Promotes and contributes to a positive work environment.


Qualifications

Job Specifications:
  • Education: A Minimum of a Bachelor of Science in social work from an accredited school of social work
  • Experience: At least one-year experience in a healthcare setting
