Job Summary
We are seeking an experienced Certified Nurse Practitioner to join our Endocrinology team at Crystal Run Health in Middletown, NY. As a member of our team, you will be responsible for providing high-quality, patient-centered care to patients with diabetes and other endocrine disorders.
Responsibilities
- Provides evidence-based medicine and adheres to established clinical guidelines.
- Develops and implements individualized care plans to meet the unique needs of each patient.
- Collaborates with other healthcare professionals to provide comprehensive care.
- Communicates effectively with patients, families, and caregivers to ensure seamless care coordination.
- Participates in quality improvement initiatives to enhance patient outcomes and experiences.
Requirements
- Master's degree in nursing
- Current, unrestricted Nurse Practitioner Medical License in the State of New York (or ability to obtain)
- Current, Active DEA license and Prescriptive Authority based on state requirements (or ability to obtain)
- Board Certification by the ANCC/AANP (Nurse Practitioner)
