Description:Schedule Options: 7.5, 10, 12, or 15-hour shifts | Full-time, Part-time, Per DiemPosition Summary:The LPN is responsible for delivering the highest quality of care and services to residents by safely administering medications and treatments, supervising CNAs, and supporting daily living activities. This role ensures compliance with Crouse Community Centers mission and policies while maintaining a safe, organized, and compassionate environment.Key Responsibilities:Medication Administration & Documentation:Administer medications and treatments using the Six Rights of Medication Pass: right resident, medication, dosage, time, route, and right to refuse.Ensure medications are passed on time and documented accurately.Record refusals or missed doses per protocol.Maintain strict confidentiality of resident information and medication records.Count and verify narcotics with another Medication Care Nurse.Resident Care & Communication:Monitor residents conditions and assess needs to provide optimal care.Review and update the Daily Log to stay informed of resident status.Report resident concerns to the Nurse Manager during medication rounds.Demonstrate and promote the mission and values of Crouse Community Center in all interactions.Supervision & Compliance:Oversee and support Certified Nursing Assistants (CNAs).Admit, transfer, and discharge residents as needed.Practice standard precautions for hygiene, cleanliness, and infection control.Ensure all care and services align with facility policies and regulatory standards.Requirements:Qualifications:Must possess a current license to practice as an LPNLong term care experienceMust be eligible to work in LTC per the NYS HCS guidelinesAbility to handle multiple prioritiesPrevious experience working with seniors preferred and desire to serve and care for seniorsAbility to make choices, decisions and act in the residents best interestAbility to react and remain calm in difficult situationsPossess written and verbal skills for effective communication and level of understandingDemonstrates good judgment, problem solving and decision-making skillsYou deserve the BEST and we are looking for the BEST Crouse Community Center is Not-For-Profit, Community Based Organization thats mission is to serve the area.
