Work Shifts

8:00 A.M - 4:00 P.M

Job Description
1. Assess patients based on:
a) Interview of patient and significant other
b) Chart review
c) Referrals from other health team members and outside agencies.
2. Provide counseling and teaching for clinic patients and their family/significant others who are new or referred by the physician.
3. Provide the appropriate nursing interventions based upon assessment which will include:
a) Instructing patients and/or family members/significant others regarding treatment, medication, diagnostic procedures and the importance of follow-up visits.
b) Interpreting the patient's understanding and emotional response to their condition.
c) Observing and identify underlying and contributing factors/concerns, especially socioeconomic, cultural, physical and language barriers.
d) Assisting patients and/or significant others to establish a medication schedule when on multiple medications.
e) Making referrals to other departments and agencies as appropriate, i.e. Home Care, Visiting Nurse Service, Department of Health, Social Service, Dietary, etc.
4. Provide health teaching and counseling to the patient and/or significant other.
5. Triage patients who present to clinic without appointments.
a) Make appropriate referrals
b) Communicate with physician or other health care members as indicated.
6. Document all observations on one or more of the following:
a) Initial Nursing Assessment
b) Follow-up check off list/flow sheet
7. Assist with hospital admission process.
a) Expedite process by communicating with necessary personnel.
b) Support patient and family members.
8. Participate in preventative health measures and case findings.
a) Maintain health maintenance status of patient by review of necessary records.
b) Diagnose human response to actual or potential health problems at the earliest signs and symptoms and initiate treatment and/or make referrals for appropriate care.
c) Identify the family members and/or significant other of patients with an infectious process and encourage testing and prophylaxis as appropriate, i.e. tuberculin skin testing, hepatitis screen.
d) Review immunization records to update status.
e) Identify those patients who are at risk for certain diseases which could be minimized with appropriate vaccines, i.e. influenza, pneumovax.
f) Conduct heath education classes to promote wellness, i.e. smoking cessation classes.
9. Communicate patient care problems/issues via the proper channels.
a) Inform Head Nurse of patients with complicated problems and those who are awaiting admission.

Responsibilities
1. A valid New York State license and current registration to practice as a Registered Professional Nurse in New York State; and
2. Holds, or obtains through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA).
Special Note:
For assignments in Post-Acute Care ONLY, individuals meeting the following criteria may also be considered for appointment: Graduation from a nursing education program registered by the New York State Education Department (NYSED) as licensure qualifying for Registered Professional Nurse (RN) and has applied for licensure as an RN. These unlicensed individuals may be employed to practice under the immediate and personal supervision of a NYS licensed, currently registered, RN for up to 90 days immediately following graduation pending receipt of a limited permit, which selected candidates must apply for and receive within the same 90-day period. Once a limited permit is received, individuals can practice nursing, under the immediate and personal supervision of a NYS licensed, currently registered, RN at a NYC Health + Hospitals health care setting for up to one (1) year from the date of issuance of the limited permit. In addition, must hold or obtain through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA).
