Licensed Clinical Social Worker/Marriage Family Therapist - Telehealth (Spanish) Department: HS - ACM Employment Type: Full Time Location: 1600 Corporate Center Dr., Monterey Park, CA 91754 Reporting To: Colleen Del Valle Compensation: $80,000 - $120,000 / year Description Astrana Health is seeking a Spanish-speaking Licensed Behavioral Health Counselor (LCSW/LMFT) to provide telehealth services to Medicaid members experiencing substance use disorders (SUD) and co-occurring behavioral health (BH) diagnoses. This is a unique opportunity to manage an acute caseload of patients who cannot receive timely follow-up care following a hospitalization for SUD or BH admissions. Your goal will be to provide timely and thorough follow-up care to patients who are high utilizers of the ER with intent to link these individuals to longer-term, sustainable solutions in managing their diagnoses. Our Values: Put Patients First Empower Entrepreneurial Provider and Care Teams Operate with Integrity & Excellence Be Innovative Work As One Team What You'll Do Conduct acute, comprehensive assessments for patients with behavior health and/or substance use disorders to support quality gap closure for Medicaid patients via telehealth appointments. Manage caseload of patients who have recently discharged from or highly utilize the ER to keep patients out of the hospital and connected to more sustainable solutions. Provide short-term individual therapy using evidence-based modalities (e.g. CBT, relapse prevention). Develop individualized treatment and recovery plans in collaboration with clients. Identify co-occurring disorders and coordinate integrated care. Facilitate psychoeducation sessions on addiction, relapse triggers, coping skills, and harm reduction. Document and complete visit notes in a timely manner, ensure coordination of care with newly assigned provider patient will transition care to. Coordinate care with psychiatrists, primary care providers, CHWs, peer specialists, and other treatment team members. Refer clients to community resources such as rehab, housing, employment, and recovery support programs. Participate in multidisciplinary case conferences and team meetings. Maintain accurate and timely clinical documentation in accordance with HIPAA, DHCS, and agency standards. Complete assessments, treatment plans, progress notes, discharge summaries, and outcome reports. Adhere to ethical and legal standards set by the Board of Behavioral Sciences (BBS) and state regulations. Advocate for client access to appropriate treatment and support services. Promote stigma reduction and culturally sensitive approaches to care. Other duties assigned. Qualifications Master's degree in Social Work, Marriage Family Therapy, or related field. Active and unrestricted LCSW or LMFT license. 2+ years of clinical experience providing services to substance use and/or mental health patients. Experience with trauma-informed care and co-occurring disorders preferred. Bilingual Spanish preferred. Environmental Job Requirements and Working Conditions This is a full-time position. Hours are M-F 8:30 AM - 5 PM. This position operates on a hybrid schedule. The office is located at 1600 Corporate Center Drive Monterey Park CA 91754.
