Post Acute Care Opportunities

We are currently seeking a skilled and compassionate Licensed Practical Nurse to join our Post Acute Care team. As a LPN, you will be responsible for providing high-quality nursing care to our patients, working closely with our interdisciplinary team to ensure the best possible outcomes.

Job Summary:

  • Provide direct patient care, including administering medications, wound care, and vital sign monitoring.
  • Collaborate with the care team to develop and implement individualized care plans.
  • Conduct thorough assessments of patients' physical and emotional needs.
  • Develop and implement strategies to promote patient independence and self-care.

Requirements:

  • Graduate of an accredited LPN program.
  • Current LPN license or compact license recognized by the State of Vermont required.
  • Must possess strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare team members.

Preferred Qualifications:

  • Previous long-term care experience.
  • Basic computer skills and ability to learn electronic medical records.
