Nurse Clinician | Western Psychiatric Hospital

Consult Liaison service is a collegial, academically minded, innovative team consisting of psychiatry and nursing serving as liaisons between medical and mental health. This particular consult-liaison site is similar to an outpatient clinic practice in which transplant recipients and donors are present and evaluated by a multidisciplinary team including a behavioral health evaluation as deemed necessary. Our team completes the behavioral health evaluation, assists with resource connection as needed, collaborates with outpatient providers and participates in a multidisciplinary selection committee in discussion of patient's readiness for transplant; all with a team of psychiatric nurses and under the direction of a top-notch supervising transplant psychiatrist. Training in this sub-specialty of psychiatry will be provided. We have team longevity with expertise recognized by multiple publications, service awards, and patient accolades.
The dress code for this position is business casual or scrubs. The schedule is Monday through Friday from 8:00 AM to 4:30 PM which ensures no holidays and no on call after hours. We offer professional development and continuing education opportunities
Responsibilities:
* Attend a minimum of 6 educational meetings offered within Institute or Outside Program
* Provides routinely scheduled staff education on clinical issues.
* Role Models professional practice
* Participates in on-going review and development and provision of Group Programming for the unit.
* Have knowledge of psychiatric diagnoses and be able to differentiate diagnosis on assigned patients.
* Participates in interdisciplinary meetings and other activities.
* Monitors psychiatric, medical, psychosocial status of assigned team.
* Maintains professional communication with multidisciplinary team members.
* Coordinates patient follow-up discharge plans and medical plans with Social Work and Primary Medicine respectively.
* Actively participates in treatment team meetings.
* Has good organizational skills that leads to effective time management.
* Attend a minimum of 2 unit based in services yearly.
* Provides education and consultation regarding psychiatric treatment, management, and psychopharmacology for staff.
* Completes documentation according to established policies and in timely fashion.
* Acts as liaison between disciplines, departments, and hospitals for assigned patients.
* Maintain competency requirements.
* Coordinates daily care for assigned team.
* Utilize up to date practices/information in delivery of patient care.
* Rounds daily on team of assigned patients and/or discuss patients with treatment team.
* Initiates or reviews admission paperwork of all assigned patients
* Provides patient/family education around psychiatric medication/diagnosis as needed.
* Coordinates and facilitates with MD and treatment team direct admissions and DEC admissions as assigned.
* Able to interface with various members of multidisciplinary team members to coordinate all aspects of patient care.
* Leads routine therapeutic patient groups.
* Assesses client from holistic perspective.
Qualifications:
* Bachelors degree in Nursing or related field required
* MSN preferred
* Two years psychiatric nursing experience preferred
Licensure, Certifications, and Clearances:
ANCC certification as CNS or ANP preferred. PA RN License required. CPR required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire.
