We are currently seeking an Independent Prescribing Pharmacist to join our friendly, multidisciplinary team. This is a fantastic opportunity to contribute directly to patient care, leading on safe and effective prescribing, managing chronic conditions, and supporting medication optimisation across our practices. Job Responsibilities Conduct patient consultations and prescribe within your clinical competence. Undertake medication reviews, particularly for patients with long-term conditions. Support the management of chronic diseases such as diabetes, asthma, and hypertension. Collaborate closely with GPs, nurses, and other healthcare professionals to optimise patient care. Deliver structured medication reviews and deprescribing initiatives where appropriate. Support antimicrobial stewardship and contribute to reducing inappropriate prescribing. Participate in audits and quality-improvement initiatives within the practice. Provide training and support to practice staff on prescribing and medication-related issues. Take a proactive role in managing long-term conditions and promoting antimicrobial stewardship. Provide expert advice on medications, ensuring safe and effective prescribing in line with local and national guidelines. Essential Requirements Registered Pharmacist with the General Pharmaceutical Council. Independent Prescriber qualification. Experience in primary care or general practice settings. Strong clinical decision-making and problem-solving skills. Ability to work autonomously as well as part of a multidisciplinary team. Excellent communication and patient consultation skills. Desirable Skills & Experience Experience managing long-term conditions. Knowledge of GP systems such as EMIS or SystmOne.
