Community Physician Network is seeking a Family Medicine physician to join our outpatient practice in Lapel, IN.
- Well established practice
- Shared call schedule with 24/7 RN triage
- Patient Centered Medical Home practice model with pharmacist, dietician, social work, etc.
- Electronic Medical Record (Epic)
- Ability to practice in a smaller community while having the support of a large network
Licensure & Certifications
- Board certified/board eligible in Family Medicine
- Licensed in Indiana or eligible for medical licensure in Indiana
