**Job Duration:** 26 Weeks
**Shift:** 7P-7A, 12-Hour Night Shift
**Guaranteed Hours:** 48 Hours per Week
**Experience:** 1 Year Minimum for RN and LPN; CNA Experience Requires Manager Approval for Candidates with Previous Hospital Experience Not Within the Last Year
**License:** RN/LPN/CNA
**Certifications:** BLS for RN/LPN and CNA
**Must-Have:** Experience Caring for Geriatrics; Charge Experience for RN
**Job Description:**
- Care for a variety of patients who are discharge-ready and awaiting community placement.
- Work with patients requiring heavy behavioral/psych support alongside long-term care/Skilled Nursing Facility care.
- Provide care under a nursing home model with a focus on fall prevention, skin integrity, and activities.
- Administer medications, including occasional IV antibiotics, and carry out dressing changes.
- Conduct daily responsibilities such as medication administration, dressing changes, and straight catheterization.
- Perform daily nursing tasks including medication scanning and chart documentation, focusing on activities, behavioral notes, vital signs, and overall patient impressions.
- Follow team nursing approach with RN patient ratios of up to 1:9.
- Engage in team support with other healthcare professionals, including CNAs, activity coordinators, and care management staff.
- Utilize the Cerner charting system and other medical equipment.
- Orientation includes two days of hospital orientation and one shift on the unit.
- Participate in an every-other-weekend rotation and holiday scheduling as needed.
- Adhere to professional dress code and appearance guidelines.
**Support on the Unit:**
- CNA assistance with a target ratio of 1:6-7.
- Unassigned charge nurse for additional support.
- On-site activities support person, social workers, and case managers.
- Access to phlebotomy for peripheral blood draws and in-house respiratory therapy.
- Collaboration with hospitalists and physical/occupational/speech therapists during day shifts.
**Floating Policy:**
- RN may float to any area within scope of practice based on needs.
- LPN floating unlikely but may occur to similar units if needed.
- CNA to float only if overstaffed to any area within scope.
