Licensed Practical Nurse (LPN) Position Details:
· Type: Per Diem
· Location: Bellevue, WA
Responsibilities:
· Understand and adhere to established policies and procedures.
· Ensure adherence to Universal Precautions for resident and staff safety.
· Prepare residents' medications for short-term absences following state regulations.
· Collaborate with the healthcare team to implement the nursing care plan for each patient.
· Initiate preventive and rehabilitative nursing procedures as appropriate.
· Observe signs and symptoms and report to the physician and RN, including reactions to treatments and changes in the patient's physical or emotional condition.
· Teach and counsel patients and their families/significant others on nursing care needs and related issues for home care.
· Participate in medical record audits as assigned.
· Attend staff meetings, patient care conferences, and in-services as scheduled.
· Read Daily Communication Logs for resident information.
· Document any changes in resident status in Progress Notes and review them regularly.
· Complete resident treatments as ordered and scheduled.
· Assist with personal care and physical support as needed.
· Performs other duties as assigned.
Other Skills or Qualifications:
· Proficient computer skills in managing the electronic health record (EHR).
· Ability to manage information electronically and transfer records appropriately.
· Ability to work in an electronic environment.
· Refined deductive reasoning skills for problem-solving during shifts.
· Must have a valid driver’s license or dependable access to a vehicle for business use.
· Must be in good health and physically/mentally capable of performing assigned tasks.
· Working knowledge of personal computing and word processing software, particularly Microsoft Windows.
· Completion of appropriate competencies as required by the facility.
Requirements:
· Licensure: Current unrestricted license to practice as a Licensed Practical Nurse (LPN) in WA state.
· Active BLS (Basic Life Support) certification.
· Preferred minimum of one (1) year of nursing experience.
· Strong communication and interpersonal skills.
· Ability to work well in a fast-paced SNF environment.
· Two (2) positive work references
· Successfully complete and pass a background check and drug test.
