**Must-Have:**
- Recent experience conducting comprehensive assessments of patients
- Two years of experience in home settings such as Certified Home Health Agency, Licensed Home Care Service Agency, or Managed Long-Term Care
- Experience conducting assessments similar to UAS such as OASIS
- Experience providing primary care to homebound individuals
- Experience in care management, including hospice and assisted living
**Job Description:**
- Perform initial in-home evaluations of patients, assessing health status, strengths, care needs, and preferences to guide the development of individualized care plans
- Review consumer medical documentation and health referral forms as relevant to the case
- Enter evaluation data into electronic evaluation form and transmit as required
- Document concerns, conflicting information, or other issues that surface during evaluation
- Emphasize continuity of care to reduce or eliminate fragmentation, duplication, and treatment gaps
- Discuss healthcare options with consumers, including supports needed, service vendor options, and waiver options
- Initiate communication between the call center and eligible consumers for enrollment into a managed care health plan
- Enhance communication and collaborative relationships with interdisciplinary care team members to improve care coordination
- Collect quality review data and any required documentation to support outcome measurements and record case notes into CRM
- Identify opportunities for health promotion and illness prevention
- Maintain comprehensive knowledge of community resources, payor requirements, and network services for the target population
- Meet all standards established for this position as outlined in the corresponding annual performance criteria and bonus template
- Perform other duties as assigned by the Regional Nurse Manager or project management
