  • Required Skills: Conduct initial evaluation, Coordinate services to home care and equipment following discharge, Coordinates with internal and external team members, Documents pertinent information to facilitate care coordination, Participates in care conference activities, Refers to other disciplines/resources, NASW Code of Ethics/Standards of Home Health Care, Care coordination, Discharge Planning

Job Details:

  • Pay: $2275.52/week - $2505.27/week
  • Assignment Length: 13-week assignment
  • Shift: 5, 8-Hour 08:00 - 16:30
  • At least one year of experience required
