Requirements:

  • Required Certifications: MSW
  • Required Skills: Facilitate discharge and contingency planning,Educates patient/caregiver to facilitate discharge,Suicide/homicide/self-harm,Psychiatric Disorders,High risk intervention patient vulnerability or safety risk,Knowledge of cultural issues,NASW Code of Ethics/Standards of Home Health Care,Complete intake and admission screenings,Participate in development of treatment plan,Perform Bio-psychosocial assessments,Provide patient and family education, support and advocacy,Work with multi-disciplinary team to ensure continuity of care

Job Details:

  • Pay: $1424.58/week - $1654.19/week
  • Assignment Length: 13-week assignment
  • Shift: 3, 12-Hour 19:00 - 07:00
  • At least one year of experience required
