**Experience:** Minimum 1 full year of Adult Home Health Experience or 1 full year of Hospice experience

**License:** Must have license in hand

**Certifications:** CPR, BLS

**Must-Have:**
- Proficiency in HomeCare HomeBase (HCHB) and OASIS, with usage in the last year
- Reliable transportation, current driver's license, and agency-required liability insurance
- Willingness to travel up to 75 miles from the home clinic

**Job Description:**
- Provide, coordinate, and direct the provision of home nursing care according to physicians' orders and agency policies and procedures
- Responsible for supervising and evaluating Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses
- Conduct an average of 5-8 home visits per day, managing various types of cases such as Start of Care, Wound Care, and Follow-up Visits
- Engage in periodic office meetings and ensure all activities are accurately documented based on the point system
- Participate in on-call rotations approximately every 6-8 weeks and as needed
- Coverage area includes Montgomery County, with expectations to meet productivity goals totaling 30 points per week
