**Job Description:**

- Compile health information by reviewing, cataloging, and checking medical records for completeness.
- Organize medical reports for placement in health information files.
- Review charts to ensure all reports and required signatures are present.
- Prepare charts for new admissions and fill out necessary forms.
- Prepare requests for specific reports or certificates.
- Compile and type statistical reports such as daily and monthly census, Medicaid days, admissions, discharges, or length of stay.
- File reports into health information records and update logs.
- Retrieve and coordinate health information records within the filing system.
- Provide information from health records after determining the appropriateness of the request.
- Coordinate with other departments concerning health information records procedures.
- Proofread medical reports and identify errors.
- Recognize when medical records information is missing.
- Gather, collate, and classify information about data, people, or things.
