Job Title: Director of Case Management

Shift: Monday-Friday, 8:00 AM - 5:00 PM
Guaranteed Hours: 40 hours per week
Experience: Minimum of 3 years in leadership with a high-volume case management program in a hospital or clinic setting
License: Current Registered Nurse Licensure
Certifications: Bachelor of Science in Nursing
Must-Have: Case management leadership experience required

Job Description:
- Assist with planning, directing, organizing, monitoring, and staffing the Case Management Department while adhering to policies, budgets, objectives, and directives mandated by regulatory agencies.
- Provide oversight of case management, discharge planning, and utilization review.
- Lead and guide management, medical staff, resource, and clinical staff in planning and conducting organization-wide case management activities.
- Participate in operations to ensure comprehensive departmental services compliant with regulatory requirements.
- Promote a collaborative departmental approach to strategic operational planning and the development of organizational policies.
- Continuously assess, measure, and improve departmental performance.
- Manage departmental resources responsibly.
- Demonstrate clinical/technical and managerial competence.
- Ensure staff professional needs are met, including education and training requirements.
- Exhibit strong leadership and managerial skills, with the ability to plan, delegate, monitor, and improve work performance.
- Maintain sound judgment, patience, and professionalism in a busy and stressful environment.
- Utilize strong interpersonal, verbal, and written communication skills.
- Apply creativity, problem analysis, and decision-making skills.
- Exercise tact, discretion, sensitivity, and maintain confidentiality.
- Demonstrate detail orientation, effective organizational skills, and the ability to prioritize tasks.
- Proficiency with standard office equipment and computer applications, such as MS Office, EMR, and internet applications.
- Understand that regular attendance is required, with potential for work beyond the standard 40-hour workweek, including weekends and after-hours shifts.
- Be prepared for on-call work to promptly address organizational, patient, or employee needs.
- Ensure readiness to adapt to occasionally required holiday coverage and possible shift cancellations due to low census or unforeseen circumstances.
