**Job Description:**

- Provide ongoing support and expertise through comprehensive assessment, planning, implementation, and overall evaluation of individual patient needs.
- Enhance the quality of patient management and satisfaction, promoting continuity of care and cost-effectiveness through case management, utilization review, and discharge planning.
- Review work lists to prioritize patients and identify new admissions.
- Conduct and document assessment and a plan of care per departmental guidelines.
- Participate in daily Care Management Touchpoints per established protocols.
- Communicate with Care Management Assistants to share priorities.
- Attend and actively participate in meetings to provide and receive information on patient progression.
- Alert care team to concerns impacting patient discharge and assist with discharge readiness.
- Modify discharge plans based on shared information.
- Assist with identifying the expected discharge date (EDD).
- Collaborate with Utilization Managers and Social Workers to discuss updates and action items.
- Attend weekly meetings to present patients and collaborate to solve issues with complex cases.
- Identify high-risk cases for escalation.
- Discuss barriers to discharge and concerns impacting care with the multidisciplinary team.
- Coordinate family meetings to support care progression.
- Educate and/or coordinate referrals to community resources and post-acute providers as needed.
- Communicate medical milestones for transition with the patient/family.
- Monitor all observation patients to ensure appropriate care progression.
- Assess discharge plans to determine post-discharge needs and communicate with all relevant parties.
- Identify required authorization for post-discharge services and coordinate with providers.
- Participate in medication resource management for non-resourced patients when needed.
- Ensure patient understanding and agreement of the discharge plan.
- Refer administrative tasks to Care Management Assistants.
- Consult with Social Workers and Utilization Managers as per departmental protocols.
