**Job Description:**
- Work in a 30-bed unit with a daily average of 100-110 patients, increasing to 120 during the winter season.
- Perform patient assessments in a timely and accurate manner according to hospital and unit policy.
- Conduct initial assessments based on the patient's clinical presentation, care setting, age, development level, and cultural and religious orientation.
- Identify and prioritize further assessment needs based on the patient's condition and response to previous care.
- Initiate discharge planning upon patient admission as appropriate.
- Develop individualized Patient Plans of Care with input from the patient, family, and multidisciplinary healthcare team members.
- Identify specific nursing diagnoses and interventions based on assessment findings.
- Effectively communicate patient care needs to all members of the multidisciplinary healthcare team.
- Conduct patient and family education tailored to the patient's age, learning needs, understanding level, and readiness to learn.
- Document nursing interventions, patient teaching, and responses to the Plan of Care accurately.
- Revise and update the Plan of Care based on patient responses and situations impacting patient/family outcomes.
- Adhere to the five rights of medication administration and maintain transcription accuracy.
- Administer blood products and IV therapy per hospital policy.
- Function effectively during emergency patient care situations and demonstrate interventions regarding the use of restraints.
- Perform waived testing following hospital policy.
- Maintain a high level of clinical expertise and adapt to new information, technology, and changing nursing practice trends.
- Maintain required certifications and demonstrate problem-solving skills in a high-volume, fluctuating census environment.
- Subject to frequent interruptions in the work environment.
