**Certifications:** BLS (Basic Life Support, American Heart Association)

**Job Description:**
- Provide direct patient care services to hospice patients and supportive care to patients' families.
- Manage an interdisciplinary team of Licensed Practical/Vocational Nurses, Certified Nursing Assistants, and support staff.
- Ensure the highest quality of care is provided and documented in accordance with the Plan of Care.
- Perform initial and ongoing assessment of patients.
- Develop and implement the Hospice Plan of Care as prescribed by the physician and collaborate with the interdisciplinary group.
- Offer all-encompassing services, including physical, psychosocial, educational, emotional, spiritual, and bereavement support.
- Report any changes in the patient's condition to both the Attending Physician and Hospice Physician.
- Maintain accurate and updated medical records, including all interdisciplinary notes and assessments.
- Practice infection control measures and standard precautions.
- Monitor staff scheduling.
- Participate in the Quality Assessment and Performance Improvement Program.
- May participate in on-call schedules.
- Attend staff meetings and complete required compliance training.
- Maintain a high level of knowledge regarding the management of pain and symptoms in the dying process.
- Make routine supervisory visits to team members at least monthly.
- Perform ongoing review and follow-up on patient clinical records to ensure quality services and compliance.
- Maintain contact with patients' families to provide information, support, and continuity of care.
- Act as the primary liaison between physician, patient/family, and hospice team.
- Educate patients, families, or caregivers as outlined in the Plan of Care.
- Evaluate care for patients and families based on nursing needs.
