- At least 2 years of acute care management experience
- Previous experience in a large academic hospital setting
- General understanding of discharge readiness
**Job Description:**
- Cover all inpatient units throughout the hospital.
- Manage a caseload of 15-20 patients or consults per day.
- Engage in medical and social discharge planning for a variety of complex patients.
- Coordinate with five local facilities with reserved beds for complex placements.
- Understand financial assistance limitations for uninsured or underinsured patients.
- Operate within a social work team consisting of full-time and part-time social workers.
- Report directly to the Social Work Manager, with oversight from the Chief of Care Coordination as needed.
- Utilize the EPIC EMR system and naviHealth for sending referrals, transitioning to EPIC Care Management over the next year.
- Participate in a 2-week department-specific orientation alongside the case management team.
- Consult with pharmacists and respiratory therapists as needed for discharge planning.
- Adhere to a dress code of any color scrubs or business professional attire.
- Rotated holiday requirements as needed.

- No weekend requirements.
- No cancellation policy listed.
- Fingerprint requirements can be completed remotely or in person prior to assignment.
