**Certifications:** EPIC experience required

**Must-Have:** Care Coordination and Utilization Management Experience

**Job Description:**

- Identify, initiate, and manage optimal patient flow/throughput to enhance continuity of care.
- Plan and facilitate coordinated, safe transitions to the next level of care required.
- Promote patient satisfaction and quality outcomes.
- Target an optimal length of stay based on the patient’s individual response to treatment, procedures, and interventions.
- Maximize contracted benefits and available services in care management planning.
- Provide comprehensive care management planning.
- Adhere to dress guidelines ensuring scrubs/apparel do not have agency or brand logos.
- Be flexible to work some holidays and weekends, specifically on Saturday and Sunday.
- Orientation includes 16 non-billable hours.

**Additional Information:**

- Prior background and license checks are required.
- Returning staff must have a one-year gap from full-time, part-time, or per diem roles to be considered as a traveler.
- Candidates currently applying for permanent roles within certain healthcare facilities or have done so in the past 6 months are not eligible.
- Comply with the policy for not contacting the facility directly.

**Health Requirements:**

- COVID-19 Vaccine required, with medical/religious exemptions considered.
- Flu Vaccine status unknown.
