Job Description

In this role, you will be responsible for evaluating patients’ nutritional status, as well as plan, coordinate and manage the nutritional care of our patients. You will also build relationships with physicians and nursing to achieve a stellar standard of care to ensure patient’s nutritional needs and goals are met.

Responsibilities

  • Conduct comprehensive nutritional assessment for patients by collecting, recording, and interpreting data from patients, family members, caregiver, and other individuals
  • Formulate nutrition intervention plan based on nutrition related concerns
  • Work together with health care providers, patient, and caregiver to implement and communicate the nutrition care plan and expected outcomes
  • Provide nutritional counseling and education to patient, families, and caregivers
  • Maintain accurate record keeping of daily clinical activities

Qualifications

  • Bachelor’s Degree in Dietetics, Nutrition, or related field
  • Registered Dietitian credentials
  • Two years of clinical/ practical experience. Geriatrics experience preferred
  • Excellent verbal and written, and oral communication skills.
