**Certifications:** CCM preferred within 18 months of eligibility.
**Must-Have:** Must be LCSW or LMSW prior to start date
**Job Description:**
- We are seeking experienced Social Workers with a minimum of 2 years' experience in acute care.

- Engage in a collaborative Care Management Triad Team Model alongside RN Case Managers and Utilization Management professionals.
- Resolve psycho-social barriers and support discharge needs for patients.
