**Experience:** Minimum 1 full year Adult Home Health or 1 full year Hospice experience required

**License:** Must have license in hand

**Certifications:** CPR, BLS

**Must-Have:** Proficiency in OASIS and HomeCare HomeBase; Reliable transportation with current driver's license and liability insurance

**Job Description:**

- Provide, coordinate, and direct home nursing care according to physician's orders and agency policies.
- Supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
- Perform home visits and attend periodic office meetings.
- Responsible for managing daily patient visits, with an average of 5-8 patients per day.
- Different visit activities, including documentation and travel, have specific point values contributing to weekly point goals.
- Types of cases handled include Start of Care, Wound Care, Follow-up Visits, etc.
- Travel up to 75 miles from the home clinic with mileage reimbursed at the IRS Standard rate; compensation is provided for patient-to-patient travel.
- Handle a territory covering Dorchester, West Roxbury, and Jamaica Plain.
- Complete all patient documentation within 24 hours of the appointment to ensure effective billing and follow-up care.
- The clinician must be open to floating between different clinics if patient assignments are insufficient.
- Dress code requires scrubs, and proficiency in HomeCare HomeBase electronic medical record system is mandatory.
