**Must-Have:**
- Recent experience conducting comprehensive assessments of patients
- Knowledge of the theory, principles, and practices of general professional nursing
- Familiarity with state and agency laws and regulations governing professional nursing practices
- Strong organizational, interpersonal, written, and verbal communication skills
- Ability to work effectively with multi-generational consumers
- Sensitivity to the concerns of others
- Capability to perform in a fast-paced, deadline-oriented work environment
- Independence in executing complex tasks
**Job Description:**
- Perform initial in-home evaluations of patients, assessing health status, strengths, care needs, and preferences to guide the development of individualized long-term care service plans.
- Review consumer medical documentation and health referral forms as relevant to cases.
- Enter evaluation data into electronic forms and transmit as required.
- Document concerns, conflicting information, or other issues that arise during the evaluation process.
- Emphasize continuity of care, reducing or eliminating fragmentation, duplication, and gaps in treatment plans.
- Discuss healthcare options, required supports, service vendor options, and waiver options with consumers.
- Initiate communication with call center personnel and eligible consumers for enrollment into managed care health plans.
- Enhance communication and collaborative relationships with interdisciplinary care team members to improve care coordination and facilitate service delivery.
- Collect quality review data and required documentation to support outcome measurements; record case notes into CRM.
- Identify opportunities for health promotion and illness prevention.
- Maintain comprehensive knowledge of community resources, payer requirements, and network services for the target population.
- Meet all standards established for this position as outlined in the corresponding annual performance criteria.
- Perform additional duties as assigned by the Regional Nurse Manager or project management.
