POSITION SUMMARY

The Certified Home Health Aide is responsible for providing personal care and related services as assigned on the Plan of Care or Plan of Treatment to preserve good patient hygiene and to promote a healthful and safe environment.

QUALIFICATIONS
  1. Current Certified Home Health Aide certificate.
  2. Minimum one-year current experience in home health or related health care setting.
  3. Knowledge of medical terminology.
  4. Current Basic Life Support (BLS) CPR Certification.
  5. Current and satisfactory report on pre-employment physical examination including TB Screening Test or chest X-ray as required by Agency policies and procedures. Must be free from signs of infection and illness.
  6. Must demonstrate competency by passing a written Competency Test or demonstrate competency in performing personal care services prior to providing patient care.
  7. Able to walk, bend, stoop, and lift objects appropriate for job performance. Must be able to perform all job responsibilities.
  8. Must be able to communicate effectively, read, write legibly, spell correctly, perform basic mathematics, understand and carry out service plans and physician orders.
  9. Must be a licensed driver with an automobile that is insured in accordance with state and/or organizational requirements and is in good working order.

ESSENTIAL DUTIES AND RESPONSIBILITES
  1. Assists patients with personal hygiene such as bathing, hair care, skin care, oral care etc.
  2. Assists patients with transfers such as in and out of bed, assists with ambulation.
  3. Assists with prescribed exercises as taught by appropriate health care personnel.
  4. Assists with the preparation of meals, including therapeutic diets and assists patients with eating.
  5. Assists patients with toileting, going to the bathroom or in using commodes, bedpans or urinals.
  6. Assists patients with medications that are ordinarily self-administered. The home health aide shall not administer medications of any kind.
  7. Performs other activities taught by a health professional for a specific patient. These may include such services as changing colostomy bags, changing non-sterile dressings, taking vital signs, and non-sterile bowel and bladder hygiene care.
  8. Reports changes in the patient's condition and needs to the Case Manager, nurse or therapist or clinical supervisor.
  9. Completes patient records regarding services performed. Records and reports any observations or changes in patient condition, patient complaints etc. Records intake and output and vital signs as ordered.
  10. Performs all assigned personal care services as per specific assigned patient Plan of Care or Plan of Treatment.
  11. Performs simple household procedures essential to maintenance of proper health care at home, such as disposal of infectious secretions, cleaning of soiled linen, proper disposal of food, light housekeeping which does not prolong stay or interfere with patient care.
  12. Attends all mandatory in-service training and educational offerings. Must successfully complete 12 hours of in-services per year.
  13. Regularly attends and participates in scheduled case conferences and staff meetings.
  14. Complies with HIPAA regulations, maintains and conserves confidentiality of patient and agency information at all times.
  15. Utilizes patient care equipment and supplies safely, efficiently and effectively.
  16. Familiarity with Title 22
  17. Maintains a comfortable work environment for all employees
  18. Maintains proper timekeeping
  19. Follows policies and procedures as per Employee Handbook
  20. Maintains all credentials up to date
  21. Reports Fraud and Abuse.
  22. Conducts timely recording and/or documentation of client contact.
  23. Conforms to all agency policies and procedures.
  24. Performs other duties that may be assigned.
