Chesapeake Regional Pharmacy is seeking a skilled and compassionate Long-Term Care Pharmacist to join our dedicated team. In this role, you will provide pharmaceutical care and support to patients residing in long-term care facilities. Your responsibilities will include medication order review, dispensing medications, monitoring drug therapy, and collaborating with healthcare professionals to optimize patient outcomes. As a key member of our pharmacy team, you will play a crucial role in ensuring the safe and effective use of medications in a long-term care setting.

Responsibilities:

Medication Order Review: Review medication orders for accuracy, appropriateness, and adherence to formulary guidelines and regulatory requirements. Conduct thorough medication profile reviews to identify potential drug interactions, duplications, allergies, and contraindications. Collaborate with healthcare providers to resolve any medication-related issues or concerns.

Medication Dispensing: Accurately dispense medications in accordance with prescriptions and established protocols. Ensure proper labeling, packaging, and documentation of dispensed medications. Conduct final accuracy checks to ensure the correct medication, dosage, and instructions are provided to the patients.

Drug Therapy Monitoring: Monitor patients' medication therapy to ensure optimal therapeutic outcomes and minimize adverse drug reactions. Perform therapeutic drug monitoring as necessary and make recommendations for dose adjustments based on patient response and laboratory values. Document interventions, recommendations, and patient progress in the electronic health record.

Collaborative Care: Collaborate with physicians, nurses, and other healthcare professionals to develop and implement comprehensive medication management plans. Participate in interdisciplinary team meetings and provide valuable input on medication-related topics. Provide medication counseling and education to patients and caregivers regarding medication administration, potential side effects, and drug interactions.

Quality Assurance and Compliance: Adhere to all applicable legal, regulatory, and accrediting agency requirements, including state and federal laws governing pharmacy practice in a long-term care setting. Implement quality assurance programs to ensure the accuracy, safety, and integrity of medication-related processes. Participate in medication error reporting and analysis, and implement corrective actions as necessary.

Qualifications:
  • Current pharmacist license in Delaware and in good standing with the state board of pharmacy.
  • Doctor of Pharmacy (Pharm.D.) degree from an accredited school of pharmacy.
  • Prior experience working in a long-term care pharmacy or a related field is preferred.
  • Excellent knowledge of medication therapy management, drug interactions, and medication safety practices.
  • Strong attention to detail and accuracy in medication dispensing and order review.
  • Effective communication and interpersonal skills to collaborate with healthcare professionals, patients, and caregivers.
  • Proficiency in using pharmacy software systems and electronic health records
