Transition Pathway Facilitator following MN Transition Framework (Project Discovery, collaborates with county resources, work coordinator, WBL, WTC)

Job Description:
This position is working with secondary students with individual needs that meet criteria for special educaiton. This includes knowledge about the MN Transition Framework, implementation of Project Discovery, collaboration with county resources, work coordinators, and school link employement opportunities. The special education teacher will collaborate with other service providers/case managers during the day to meet student needs.

License & Experience:
  • Licensed Special Education Teacher in the State of Minnesota
  • Knowledge and experience working with students with special education needs.
  • Experience working with secondary students and families

Essential Skills:
  • Knowledge and experience completing due process and follow timeline requirements of special education
  • Experience completing Part B evaluations
  • MN Transition Framework/Secondary Transition experience
  • Must be able to assure and implement state and federal laws, statutes, rules and regulations pertaining to special education
  • Team oriented approach when working with students, colleagues, families, and administrators
  • Effective communication and teaming skills, both verbally and in writing, with students, parents, teachers, administrators, community and staff
  • Establishes a culture for learning by supporting the mission, beliefs and the strategic plan of the district
  • Inspires and engages all students in relevant learning
  • Maintain confidentiality of information about students, staff and families
  • Experience working with diverse students and families - experience/training with CLR (Culturally Linguistically Responsive) methods preferred
