Full Time Mental Health Case Manager

Mahajan Therapeutics is looking for hard-working and conscientious Case Manager to perform various administrative tasks with a keen eye for detail and works under the supervision of the Case Manager Supervisor and Clinical Director. The Case Manager's primary job function is to enhance the overall treatment process by assisting therapists to meet the overall treatment needs of clients. Performs other duties as assigned.

Essential Duties & Responsibilities:
  • Act as a liaison between therapist, the referring agency, and family of the client. Arrange family visits, coordinate transportation, and ensure clients' needs are met.
  • Ensure communication between family, referral source and Mahajan therapeutics is agreeable and ongoing.
  • Attend court hearings as deemed appropriate.
  • Ensure documentation occurs according to policy.
  • Attend treatment staffing with prepared report that accurately represents client's response to treatment.
  • Facilitate individual and group CSPT as assigned.
  • Function on agency quality assurance or other committees as assigned.
  • Assist with in-service training as directed by Clinical Director or CM Supervisor.
  • Attend supervisory sessions with prepared agenda.
  • Ongoing assessment of client needs
  • Assistance in achieving personal independence in managing basis needs as identified by the client and/or parent or guardian
  • Facilitation of further development of daily living skills, if identified by the client and/or parent or guardian
  • Coordination of the ISP, including:
    • services identified in the ISP
    • assistance with accessing natural support systems in the community
    • linkages to formal community services and systems
  • System monitoring
  • Coordination and assistance in crisis management and stabilization
  • Advocacy and outreach
  • As appropriate to the care of clients, education and training specific to the individual needs, abilities and readiness to learn
  • Mental Health interventions that address symptoms, behaviors, thought processes, etc., that assist a client in eliminating barriers to seeking or maintaining education and employment
  • Activities that increase the client's capacity to positively impact the client's environment
  • Adequate knowledge and ability to navigate EHR (Electronic Health Record) system.
  • Assume additional duties as assigned by Clinical Director or CM Supervisor.

Qualifications:
  • Education: Bachelor's degree in Social Work, related field, or minimum of two years college credit plus five years of work experience in the field of Human Services or QMHS training adequate to perform job duties according to Ohio Law.
  • Required Licensure: QMHS
  • Experience: Experience working with adolescents, teens, and adults, emotionally disturbed population, mental health experience is preferred.
  • Interpersonal Skills: Interact in a pleasant and professional manner with other employees, physicians, providers, patients, and patient's families to promote a team effort through cooperation and respect for fellow employees.
  • Essential Technical/Motor Skills: Organizational skills, writing, speaking clearly, answering telephone, fine dexterity, handling, and gripping.
  • Essential Mental Abilities: Calculating, interpreting information, analyzing, assessing, evaluating, explaining, teaching, attention to detail, memory and reasoning.
  • Essential Sensory Requirements: Ability to see, hear, feel, and verbalize.
  • Age-Specific Care Criteria: The employee must demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on his or her assigned unit. The individual must demonstrate knowledge of the principles of growth and development over the life span and possesses the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs, and to provide the care needed as described in the unit's policies and procedures.

    • Guideline for defining age specific groups:
    • Infant: Birth to 24 months Child: 2-11 years

    • Supervision: This position has no supervisory function.
    • Proven competency with MS Office applications, including Word, Excel and PowerPoint required.
    • Adaptability in a challenging and dynamic environment; understand and work well in a matrixed organizational structure required.
    • Satisfactory references from employers and/or professional peers and satisfactory criminal background check.
    • Satisfactory drug screen
