The Physician will: 1.Be a responsible and highly qualified provider who can deliver exceptional medical care to community patients, focusing on examination, disease diagnosis, and treatment. In addition toproviding medical treatment, you will also play a vital role in preventive healthcare and promoting healthy habits, and assure the continuity of care for those patients assigned. 2.Provide professional services, shall be consistent with the requirements in place at the time for all applicable federal, state, and local regulatory entities (including, without limitation, the joint commission). 3.The referral of and coordination of care with any required specialty services as needed. 4.Participate in utilization management, quality improvement, patient satisfaction programs and committees as may be reasonably requested by the Family health center from time to time. 5.Participate in community driven outreach programs or other activities as may be reasonably requested by the Family Health Center from time to time. 6.Participate in rotation of late day, and on call coverage as per family Health Center coverage needs. 7.Provide and perform such other services as may be reasonably requested by the Center. 8.Participate in the oversight of medical, and NP students. Education and Experience requirements 1.Medical Degree (MD or DO) with residency in internal medicine with focus on adolescent to adult care (12 and up).

The Physician will:
