Summary:

The Pharmacist-Oncology Program Specialist provides clinical oncology pharmacy services and oversees the daily dispensing of anti-neoplastic therapy in the infusion room. Primarily works at a station in the infusion room and is available for questions, dose adjustments, hypersensitivity reaction response, and to coordinate efficient dispensing of the oncologic and non-oncologic treatments administered in the infusion center and outpatient infusion center. Provides direction for the development, maintenance, and integration of chemotherapy and non-oncology protocols within the electronic health record. Participates in the Cancer Center research program, overseeing the pharmacy portion of the clinical trial protocol, investigational product dispensing, and site activation meetings. Attends oncology patient rounds, infusion room huddles, and other relevant cancer center committees and meetings as assigned. Acts as the Pharmacy representative for Cancer Center Quality Programs.

Essential Duties and Responsibilities

  • Participates in the development and maintenance of electronic chemotherapy and non-chemotherapy infusion orders sets in the electronic health record (i.e., Beacon/Epic) including development of safety assessments, incorporation of standardized premedications as required, and developing standardized processes for infusion drug management.
  • Serve as a clinical liaison with the Specialty Pharmacy Services business office, retail pharmacies, and infusion clinics
  • Pharmacist-Oncology Program Specialist works collaboratively with clinical and administrative staff internally and externally to process oral medication prescriptions at the Cancer Center. Pharmacist-Oncology Program Specialist is responsible for all steps of the workflow from prescription to delivery of medication to the patient
  • Participates in the planning, execution, and safety of clinical research protocols by providing input on pharmaceutical services, assessment of participant needs, and determination of appropriate services for each clinical research trial.
  • Reviews and provides feedback on new, upcoming and pending studies in collaboration with the director of research.
  • Assists in the development and validation of research order sets in the electronic health record.
  • Supervises investigational product accountability and oversees dispensing of investigation product for oncology clinical trials.
  • Serves as pharmacy representative to review protocol modifications, participate in site initiation visits, and collaborates in the preparation for audits.
  • Updates and creates treatment plans in applicable computer based programs (i.e., beacon/EPIC)
  • Partner with physicians and other providers to provide clinical pharmacy services and to promote evidence-based, safe, effective, and accessible drug therapy
  • Evaluate appropriateness of chemotherapy orders which includes extensive profile and lab reviews
  • Prepares and dispenses infusion center orders according to established policies, procedures, and protocols.
  • Reviews medication orders and maintains accurate, complete patient medication profiles.
  • Compounds and dispenses pharmaceuticals, including sterile, chemotherapy, and parenteral nutrition preparations accurately.
  • Provides clinical oncology pharmacy services per established protocols.
  • Ensures safe, appropriate, cost-effective medication therapies for patients according to established policies, procedures, and protocols.
  • Monitors drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, drug-laboratory test interactions, allergies, appropriateness of drug and dose, and therapeutic duplications.
  • Collaborates with oncology provider staff to determine optimized anti-neoplastic therapy regimens and supportive care based on national guidelines and patient factors.
  • Reviews/interprets lab results and recommends dose changes as needed.
  • Reviews and adjusts drug regimens based on renal function according to TFHD protocols.
  • Reviews literature, national guidelines and other valid sources to determine appropriateness and accuracy of treatment plans.
  • Transitions medications from IV to oral, according to TFHD protocols.
  • Detects and reports suspected adverse drug reactions and medication errors accurately and in a timely manner.
  • Sustains the formulary by minimizing non-formulary procurements, utilizing therapeutic substitution protocols, and promoting rational drug therapy selection.
  • Provides clinical consultation and clarification to practitioners. Suggests appropriate, cost-effective therapeutic alternatives to medical staff, as needed.
  • Provides drug information to patients and their families.
  • Documents all clinical activities and interventions accurately and completely.
  • Participates in the quality improvement and medication use review activities of the department. Collects data; conducts quality monitors and inspections; and maintains logs, records, and other documentation as assigned.
  • Participates in the development and presentation of orientation, education, and training programs to the pharmacy, medical, nursing, and other staffs.
  • Contributes to the quality and effective operation of the department.
  • Supervises and directs pharmacy support personnel. Verifies the daily activities of pharmacy technicians. Participates in the performance appraisal of pharmacy support personnel.
  • Works independently with minimal supervision. Organizes and prioritizes work assignments. Ensures pharmacy services are provided in a timely manner.
  • Keeps pharmacy areas and equipment clean, neat, and well-organized.
  • Maintains competence required for current job title/position.
  • Maintains current pharmacist licensure.
  • Attends pharmacy staff meetings.
  • Attends orientation, education, and training programs. Reviews literature and other materials pertinent to the practice of pharmacy.
  • Completes all competence/skills assessment requirements.

Minimum Education/Experience

Bachelor's Degree and/or a Pharm. D. from a ACPE accredited institution and 10+ years of experience required.

Required Licenses/Certifications

California Pharmacist required upon hire.

Nevada Pharmacy Board Certificate (if primary location is California, employees hired into this job classification have ninety (90) days from date of hire to obtain a Nevada
license. Current employees in this job classification as of 1/1/23 will have until 1/1/24 to obtain their NV license).
