The Leominster Adult Outpatient Center provides strength-based individual and group therapy to adults, and couples age 18 and over. We offer integrated, whole person care with primary care, a wellness program, and behavioral health. The Leominster Counseling Center also offers specialized substance use disorder treatment. Our Office Based Opioid Treatment offers buprenorphine and vivitrol treatment and we have a Second Offender Alcohol Education program. Responsibilities:

* Complete medical and wellness assessments of patients


* Provide individual and group health and wellness education including but not limited to medication, chronic disease management, stress reduction, nutrition and other general health and wellness concerns.


* Monitor and administer medication as prescribed.


* Work collaboratively with Primary Care practitioners and Behavioral Health team.


* Provide appropriate health care and coordinate referrals.


* Care coordination of patient care and needs within the agency and all external providers, families, residential programs, etc.


* Document in electronic health records per CHL policy and procedure.


* Attend all required meetings and trainings.


* Stay current with nursing practice.


* Perform other job-related duties as assigned.

Qualifications:

* Grade N02: Community LPN: Current Massachusetts LPN license


* ASN preferred.


* Experience with quality improvement process to improve client care and meet required health risk outcome measures.


* Excellent communication and documentation skills


* 2-3 years of medical nursing experience with behavioral health and substance abuse experience preferred.


* Interest and knowledge about health and wellness, especially regarding clients with medical and behavioral health concerns


* Work well independently and as a member of and Integrated Health Care team.


* Bilingual Preferred


* Current valid US-issued driver's license and ability to provide registered and insured automobile for work related purposes including transporting clients.
