Position Title: Health Education Coordinator/IBCC Program Hourly Rate: $22.00

POSITION SUMMARY:

With minimal direction and supervision, performs health education activities with voluntary and official health agencies; interprets and coordinates to community, professional and lay groups, and evaluates public health education programs in an area served by a local health department. This position coordinates community efforts for capacity building as it relates to population health and system changes for assigned Clinical Services and Systems Programs.

SUPERVISION EXERCISED:

Provides no direct supervision of Health Department staff.

SUPERVISION RECEIVED:

The position is essentially self- directing with minimal supervised by the Director of Clinical Systems and Services.

ESSENTIAL FUNCTIONS:

* Assesses individual and community needs for planning and implementing health promotion and education activities.
* Plans for effective health education programs, acts as a resource person in health education, coordinates the provision of health education services.
* Perform needs assessments to determine existing gaps and barriers in community services, evaluates the effectiveness of various health education methods and techniques.
* Communicates health and health education needs, concerns, and resources.
* Implements health education programs in various community settings.
* Facilitate linkages with medical providers to educate about PHS programs and other community resources, and to assist clients and health department staff with accessing these resources as needed.
* Participate in regular meetings with state and UIC staff, submit regular reporting to the state.

Job Requirements

JOB REQUIREMENTS AND QUALIFICATIONS:

A Bachelor's degree with a minimum of 30 semester hours in health education with areas of concentration in the behavioral and biological sciences, AND

* A working knowledge of the psychological and sociological concepts governing individual and group behavior. This includes an understanding of the health education principles governing program planning, including Medicaid/Medicare, HMOs and Federally Qualified Health Centers.
* An ability to conduct outreach/in-reach strategies and navigate clients through health system for barrier reduction, tracking and monitoring for outcomes and cultural competences.
* A working knowledge of Microsoft 365 Office Systems, and a willingness to learn other data entry programs as necessary.
* Strong organizational skills, the ability to present ideas effectively, both orally and in writing.

ESSENTIAL PHYSICAL/COGNITIVE ABILITIES

* The ability to sit, flex, stand, reach, and bend.
* The physical strength to lift, carry, and transport materials/equipment are equivalent to thirty (30) pounds.
* Visual acuity, auditory ability, and clear written and verbal communication ability.
* Manual dexterity and the ability for data input into various computer programs.
* The ability to maintain professional behavior at all times.

SPECIAL REQUIREMENTS

* Must reside within St. Clair County, IL or be willing to relocate to St. Clair County within six (6) months from hire date.
* Must have a personal cell phone to download County IT Security Application onto for IT credentials.
* The ability to work necessary hours to meet the position requirements including evenings and/or weekends as assigned.
* May require provision of services at off-site locations.
* A valid Driver's License.
* A dependable automobile for use in position.
* Auto insurance in effect with at least the minimum limits of liability as outlined and required by the state of Illinois.
* Adherence to all local, state and federal regulations pertaining to OSHA guidelines.
* Health Department employees:
* Must be current on CDC Healthcare Personnel Vaccine Recommendations, which include Hepatitis B, MMR (Measles, Mumps Rubella), Td/Tdap (Tetanus/Diphtheria/Pertussis), Varicella (chickenpox) and Meningococcal if over 65. Influenza and COVID vaccines are recommended and encouraged, but not required at this time and maybe subject to change.
* Are required to have a TB test upon hire and separation of employment.


* All employees must adhere to the St. Clair County Personnel Code.
* Position contingent upon continued grant funding.
