Description

Zepf Center has been serving the Lucas County community for nearly 50 years. We are the leading provider of behavioral health and substance use disorder services in Northwest Ohio. Services include adult and child psychiatric, substance abuse, case management, residential, Crisis Care, and therapy programs, as well as career development and wellness services. Zepf Center also offers primary care medical services to our patients to contribute to their continuum of care. Zepf Center is a trauma-informed agency and environment for both patients and staff.

We are currently seeking an LPN Nurse Coordinator in the Withdrawal Management unit to plays a pivotal role in patient care and assumes responsibilities in a management capacity. This position involves: working, (4) shifts monthly, and (1) weekend to ensure the seamless operation of the facility and the well-being of patients.

Essential Duties and Responsibilities:
  • Patient Screening:
    • LPNs perform initial and ongoing screenings of patients to determine their physical and mental health status, substance use history, withdrawal symptoms, and any co-occurring medical conditions.
  • Medication Administration:
    • LPNs administer prescribed medications to manage withdrawal symptoms, treat co-occurring health conditions, and support the patient's recovery. This includes maintaining accurate medication records.
  • Monitoring and Observation:
    • LPNs closely monitor patients for signs of withdrawal, adverse reactions to medications, and any concerning changes in their physical or emotional state. They also document and report any observed changes.
  • Vital Sign Monitoring:
    • Regularly measure and record vital signs such as blood pressure, heart rate, respiratory rate, and temperature to assess a patient's overall condition.
  • Assisting with Activities of Daily Living (ADLs):
    • Help patients with their daily hygiene, mobility, and self-care needs, especially if patients are experiencing physical or mental health challenges during detoxification.
  • Health Education:
    • Educate patients about their condition, medication, and the importance of adhering to their treatment plans. LPNs may also provide information about harm reduction strategies.
  • Crisis Intervention:
    • Intervene and provide support during episodes of patient distress, agitation, or crisis. This includes de-escalation techniques to ensure a safe environment.
  • Collaboration:
    • Work closely with the multidisciplinary team, including physicians, nurse practitioners, registered nurses, social workers, counselors, patient care techs and others to discuss patient progress, medication adjustments, and other aspects of patient care.
Supervisory Responsibilities
  • Patient Care and Screenings:
    • Ensure orders are accurately entered into the patient record.
    • Regular evaluation of patient progress and work within Zepf and other agencies to discharge/transfer to next level of care.
  • Team Leadership and Supervision:
    • Provide leadership and guidance to the shift LPNs, Patient Care Techs and Medical Assistants (MAs)
    • Oversee the coordination of patient care and assignments during shifts.
    • Participate in training new staff, completing bi-weekly individual supervisions and developing effective workflows and procedures.
    • Assist with staff recruitment, screening, hiring, training, evaluation and disciplinary action LPNs within the department.
    • Perform other duties as assigned.
  • Medication Management:
    • Maintain accurate records of medication administration and inventory.
    • Ensure compliance with medication protocols, safety measures, and disposal procedures.
    • Collaborate with the pharmacy for medication procurement and management.
  • Adherence to Protocols and Procedures:
    • Ensure adherence to detoxification protocols and procedures.
  • Shift Management:
    • Organize and lead nursing shifts effectively, ensuring a smooth and safe operation.
    • Participate in on-call rotation between Withdrawal Management and CSU including rotation of coverage for withdrawal management unit.
  • Quality Assurance:
    • Conduct chart reviews and audits to ensure documentation accuracy and compliance.
    • Assis with completion of Prior Authorizations
    • Monitor and complete medication room audits.
    • Ensure transfer/day of discharge billing is completed in a timely manner

Position Qualifications:
Required
  • Licensed Practical Nurse (LPN) in good standing and can administer medication.
  • Professional experience related to psychiatry and substance use disorders.
  • Must have valid driver's license and insurance.
Preferred
  • Experience with supervisory responsibilities
At least 2 years' experience working with medication assisted treatment and withdrawal management.
