Job Description
Job Title: Licensed Practical Nurse (LPN) - Full Time Evenings

Job Specialty: Long-Term Care (LTC)

Job Duration: 13 weeks

Shift: Night - 5x8 - 11:00 PM - 7:30 AM - 40 Hours

Guaranteed Hours: 40 hours per week

Experience: 1 year experience as an LPN in a long-term care facility

License: NY LPN Licensure

Certifications: Basic Life Support (BLS)

Must-Have: Ability to provide care to geriatric residents

Job Description:

- Demonstrate knowledge and skills necessary to provide age-appropriate care for patients on the assigned unit.
- Display understanding of growth and development across the lifespan, with the ability to assess and interpret data to identify patient-specific needs.
- Deliver direct and indirect skilled nursing care to assigned residents.
- Follow defined policies and procedures under the supervision of a Registered Nurse.
- Maintain collaborative relationships with nursing staff, physicians, and ancillary staff.
- Keep current with nursing knowledge and clinical expertise through continuing education and in-service programs.
- Collect data on residents' physical and psychosocial conditions and communicate findings to the RN and Interdisciplinary Team.
- Contribute to a developmentally appropriate and culturally relevant plan of care, involving residents/family and healthcare team.
- Recommend revisions to the care plan after evaluating resident outcomes and facilitate referrals where necessary.
- Ensure safety, cost-effectiveness, and impact considerations in care planning and delivery.
- Participate in staff mentoring/orientation and demonstrate leadership in a professional practice setting.
- Reasonably anticipate occupational exposure to bloodborne pathogens; tasks may involve contact with blood, body fluids, or tissues.
