GENERAL DESCRIPTION

Young Adult Clinician is an innovative position with two major components: to meet young adults where they're at, to help them gain skills and confidence as they move toward independence, and to provide trauma-informed therapy services. This position provides home- and community-based services and includes activities such as hands-on coaching in independent living skills and problem-solving, individual therapy, and developing and facilitating trainings. Working alongside Independent Living Specialists who provide case management services, the Young Adult Clinician provides "do for, do with, cheer on" support for youth. The Young Adult Clinician also carries a caseload of traditional therapy clients to help them manage mental health symptoms and heal from traumatic stress and adversity.

POSITION SPECIFIC:

  • Demonstrates agency clinical philosophy in all aspects of program activities.
  • Leads youth in developing independent living skills in the areas of personal care and health, including sexual health and reproductive rights options; education; employment; food management; money management; home management; housing; transportation, community resources, and recreation; and social-emotional skills.
  • Create and facilitate quarterly psychoeducational trainings for youth groups; topics chosen in consultation with the Director of ILO based on youths' needs and the program contract.
  • Performs a comprehensive range of clinical duties including gathering of social history information, formulation of assessments and treatment plans, and implementation of treatment modalities through individual, family, and group therapy.
  • Utilizes evidence-based practice models that meet the various clinical needs of clients on assigned caseload.
  • Demonstrates a working knowledge of Medicaid Community Mental Health Services Parts 140 and 132.
  • Completes required Kaleidoscope, Medicaid, DCFS and court-required documentation within established timelines and procedures.
  • Consults regularly with program specialists, case management teams, and other collaterals regarding service planning and service delivery updates, feedback, and recommendations.
  • Participates in conferences, court hearings, team meetings, staffings and other meetings in the community as required as an advocate for clients or as a representative of the agency.
  • Travel to home- and community-based sessions.
  • Completes other duties as assigned by the Director of Clinical Services.

EDUCATION REQUIREMENTS/GENERAL QUALIFICATIONS:

  • Minimum Degree Required: Master's degree in social work or social service-related degree
  • Minimum Years of Experience: 2 years of social service experience required
  • Licensure: LCSW or LCPC licensure eligibility
  • Certification: IM+CANS Certification (can be completed upon hire)
  • Information Technology Skills Required: Microsoft Office Proficiency
  • Excellent interpersonal, oral and written communication and time management skills.
  • Full time use of minimally insured operating automobile and valid driver's license. Must pass a satisfactory Motor Vehicle Report (MVR).
